Default Merc 200 Saltwater runs rough after slow runs

My merc 200 saltwater 91 model doesnt like cruising around slowly. After a prolonged run at 5 knots (eg, up a river) if you then want to go up to about 8 knots it shakes and sounds like its running on 3 cylinders. If I take it for a burst at 15 knots or above it gradually fixes itself. My questions are: Is it fouling up the plugs as I suspect? is there a different type of spark plug that is more suited for slower revs? What else could cause this?
Once I was waiting near a boatramp and left the motor idling for maybe 20 minutes, then when the car came i went to take off and it stalled & wouldnt start for ages. Apart from that time (will never idle for so long again) and after slow runs, its runs like a dream and always starts first time.
Any help appreciated.

what oil r u running
when is the last plug change?

last plug change approx 15 months ago.
Oil is shell nautilus premium. (TC-W3 self mixing)
